Please read the Student Handbook before completing the application. You will be asked to agree to the terms and conditions outlined in the Handbook. Updates are currently being made to the Student Handbook. Please call the school for more information. Thank you for your patience and please forgive us for any inconvenience.
After reading the Student Handbook, fill out the online application below. All information must be complete and factual or application will be voided out.
Once the application is filled out, click the Submit button at the bottom of the screen to submit the application online. Clicking on Cancel will cancel the online submission of your application.

Requirements
Phoenix Academy does not discriminate against any student on the basis of ethnicity, religious affiliation, national origin, gender or disability.
If spaces are available, students will be admitted in the order in which their application was received. If applications exceed capacity, a lottery will be held. The lottery will consist of a public meeting, administered by the administration or a volunteer. Specific information is provided on the school calendar.

Students entering kindergarten may only apply if the child will be 5 years of age by August 31st of the upcoming school year.
