After reading the Student Handbook, fill out the online application below. All information must be complete and factual or application will be voided out.

Once the application is filled out, click the Submit button at the bottom of the screen to submit the application online. With some browsers, you may see a warning popup letting you know the form is being submitted by email. Please click OK to complete the submission. Clicking on Cancel will cancel the online submission of your application.

Requirements

Phoenix Academy does not discriminate against any student on the basis of ethnicity, religious affiliation, national origin, gender or disability.

If spaces are available, students will be admitted in the order in which their application was received. If applications exceed capacity, a lottery will be held. The lottery will consist of a public meeting, administered by the administration or a volunteer. Specific information is provided on the school calendar.

• Students entering kindergarten may only apply if the child will be 5 years of age by October 16th of the upcoming school year.
